Payment Plans |
|
Payment for sessions is
due at the time of the sessions. |
|
|
|
If your financial
resources are limited and you have difficulty making payment at the
time of the sessions, a payment plan can be arranged to spread the
cost of your treatment over a period longer than the course of
treatment. |

Insurance |
|
|
To use your health
insurance, plan to see me at the Massachusetts General
Hospital in Charlestown.
I DO NOT bill
insurance companies through my private practice. I will
provide a receipt for all services which you may then submit
to your insurance carrier for reimbursement. Be sure to
confirm with your insurance carrier that this documentation is
sufficient for you to receive reimbursement. With some
carriers and policies, it is not.
Many insurance
policies cover the mental health/behavioral health services I
provide at the Massachusetts General Hospital HealthCare
Center at Charlestown. However, the
hospital limits services to individuals living in Charlestown
or who have a primary care provider at the Massachusetts General Hospital
HealthCare Center at Charlestown..
If you living in
Charlestown or who have a primary care provider at the Massachusetts General Hospital
HealthCare Center at Charlestown. and wish to find
out if your insurance policy is one of these, call the
customer service number on the back of your insurance card and
confirm that your insurance policy will reimburse for "out-patient
mental health services provided by a licensed psychologist on
the staff of the Massachusetts General Hospital".
DO NOT use my name when inquiring about coverage at Mass
General as the hospital does not use individual provider names
when billing. My name may appear on insurance company
provider lists due to past contracts that are no longer
active. Using my name will confuse the authorization
process. |

Questions to Ask your
Insurance Company |
|
|
Many insurance
carriers will not pay for sessions that are provided by a
psychologist outside of their provider network and/or may
require prior authorization of sessions and/or specific forms
to be completed. Before scheduling an appointment,
please be sure to call your insurance carrier and ask the
following questions: |
|
|
|
|
Is there coverage in my
policy to pay for outpatient mental health or behavioral health services? |
|
|
Is prior authorization
by the insurance company required for payment to be made? |
| |
|
If prior authorization
is required, what are the procedures to obtain this initial
authorization? |
| |
|
How many sessions are initially
authorized for payment? |
| |
|
What
are the procedures for authorizing continuing treatment beyond the
initial number of sessions authorized? |
| |
|
Does your policy
reimburse the full amount of the providers fee for services? |
| |
|
Is there a co-payment
for services and if so what is the amount? |
| |
|
Is there an annual deductible
that must be met before the insurance carrier will reimburse for
services? |
| |
|
What information does
the insurance carrier require to authorize sessions? |
|
|
Does use of my
insurance policy allow the insurance company to review my doctor
medical records regarding my treatment? If so, who would
have authorization to review my records? |
|
|
|
Will the insurance
carrier provide information about my health history to others
including other insurance companies? |
